Instructions: Please type in your information on the renewal
form, be sure to save a copy for your records, print a copy for
either Jody Harrison or Barb Kay, and submit this renewal form
to them via the Highland Community School District mail system.
LPDC will review your certificate renewal plan and notify you
of its decision within 60 days after receipt of your plan. Our
LPDC members are Barb Kay and Jody Harrison, co-chairpersons,
Gina Hirsch, Sue Monken, and Sharon Rinderer.
